As part of the organization of practical training for third-year medical students, you are required to complete this form in order to indicate and rank your preferences regarding the training location.

- Responses cannot be edited after submission.
- In case of an error, do not submit the form again. You must contact the administration directly to have the error corrected.
- Any student who does not register or does not complete the registration process will be fully responsible for their situation. The Annex will automatically determine their training location.
- The student’s choice is not final. In case of high demand for a particular institution, the administration may assign the student to the nearest institution.
- In case of duplicate registration, both registrations will be completely cancelled, and the administration will determine the student’s training location.
